Good Faith Estimate (GFE) Disclosure

You have the right to receive a Good Faith Estimate explaining how much your therapy or mental health care will cost.

Starting in 2022, the No Surprises Act gives you protections against unexpected medical bills. If you are uninsured or choose not to use your health insurance, I am required to provide you with a written estimate of the costs of your care before your visit.

This estimate includes:

  • A description of the therapy services to be provided

  • An itemized list of expected charges for each service

  • Provider information (name, credentials, location)

  • A note that actual charges may vary

  • Your right to dispute charges that are substantially higher than the estimate

You can request a Good Faith Estimate before scheduling a session, and it’s important to save a copy for your records. For more information, visit www.cms.gov/nosurprises or call the Help Desk at 1-800-985-3059 (TTY users: 1-800-985-3059).

Providing a GFE helps ensure you can enter therapy with clarity, confidence, and trust about the cost of care.
